Single column plate plus other column lag screw fixation vs. both column plate fixation for anterior column with posterior hemitransverse acetabular fractures – a biomechanical analysis using different loading protocols

چکیده
ObjectivesOpen reduction and internal fixation of both columns is considered the treatment choice for displaced anterior column with posterior hemitransverse (ACPHT) fractures in non-geriatric patients. Plate one combined lag screw other allows to decrease operative time approach-related morbidity compared conventional plating. The aim this biomechanical study was evaluate whether single plate plus confers similar stability fixation. Physiological loads were simulated using single-leg stance (SLS) as well sit-to-stand (STS) loading protocols.MethodsA clinically relevant ACPHT fracture model created fourth-generation composite hemipelves. Fractures stabilized three different constructs: (1) (AP+PCS), (PP+ACS) (AP+PP).Specimens loaded from 50 750 N a ramp 100 N/s. Fracture gap motion (FGM) relative interfragmentary rotation (RIFR) between main fragments assessed under an optical 3D measurement system.ResultsSTS generally resulted higher mean FGM RIFR than STS AP+PCS AP+PP groups, while no significant differences found PP+ACS group. Compared (AP+PP), displayed significantly iliac wing during SLS loading. No identified group.ConclusionOverall, conferred From clinical point view, appears be most attractive alternative fractures.

Single column locking plate fixation is inadequate in two column acetabular fractures. A biomechanical analysis
BACKGROUND The objective of this study was to determine whether one can achieve stable fixation of a two column (transverse) acetabular fracture by only fixing a single column with a locking plate and unicortical locking screws. We hypothesized that a locking plate applied to the anterior column of a transverse acetabular fracture would create a construct that is more rigid than a non-locking p...
متن کاملPosterior column acetabular fracture fixation using a W-shaped angular plate: A biomechanical analysis
OBJECTIVE The purpose of this study was to compare the stability and feasibility of four fixation constructs in a posterior column acetabular fracture: one reconstruction plate, one reconstruction plate and lag screw, two reconstruction plates, and a W-shaped acetabular angular plate. METHODS Twenty embalmed cadaveric pelvises with a posterior column acetabular fractures were allocated to one...
متن کاملPercutaneous fixation of anterior and posterior column acetabular fractures.
Although open reduction and internal fixation (ORIF) has been the standard of care for acetabular fractures, recent advancements in minimally invasive techniques have allowed percutaneous fixation to gain popularity. Percutaneous technique has been described in the literature as an adjuvant to ORIF. However, isolated percutaneous fixation has the advantage of limiting soft tissue disruption, le...
متن کاملPosterior acetabular column and quadrilateral plate fractures: fixation with tension band principles.
Acetabular fractures can be classified into 5 simple and 5 associated fracture patterns. A significant amount of variation and complexity exists in these fractures patterns. Fractures of the posterior wall are the most common acetabular fractures. Comminution of the quadrilateral plate adds to fracture instability, and more rigid and stable internal fixation is mandatory. The goal of this study...
